Patients can be referred for admission to Barlow Respiratory Hospital from any medical facility in Southern California. We are recognized for nearly a 60% success rate of weaning patients off ventilators, one of the highest rates in the nation. We partner and coordinate care with admitting physicians and hospitals to ensure continuity of care. Physicians can refer patients to Barlow Respiratory Hospital for our specialized care even if they do not have an existing affiliation.

To make a new referral to admit a patient to any Barlow Respiratory Hospital location, download this form, complete and fax to us at 213-250-7415 or contact our centralized Admissions Department 213-202-6878 and provide the following:

• History and Physical (H&P)
• 3 days of Progress Notes
• Labs (current)
• 24 hour MAR
• Consultation(s)
• X-Ray report(s)
• Respiratory Reports and Setting
• Facesheet
• Insurance Card
